OCIA Registration

Welcome to OCIA at Holy Trinity Catholic Church.

We are grateful that you are taking this step in your journey of faith. This form helps us learn a little about the person being registered, their sacramental background, and how we can best walk with them or their family through the OCIA process.

Please complete one registration form for each person being registered, including each child. If you are registering a child or another family member, please answer the first questions using that person's information. Parent, guardian, and contact information will be collected later if needed.

If you are unsure how to answer a question, choose the best option available. Our OCIA team will review the registration and follow up if we need anything else.

Please do not worry if every situation does not fit perfectly into the form. We are here to help.
